Initial Assessment – Without Delegation

Prior to resident’s admission to an Adult Family Home, Boarding Home or Assisted Living facility, an assessment is done – this determines if the facility is able to provide the care and services for the resident. For DSHS clients, it is done by the Social Worker or a Case Manager assigned to the resident. Private pay residents and their representative must have a Nurse Assessor to do the assessment at their own expense.
It is a visual “head to toe” inspection/interview with the patient and their representative, a 25-27 pages that includes the patient’s demographic data, history, medications, interpersonal and psychosocial behaviors, daily routines, likes/preferences and dislikes, activity assistance need and multi disciplinary need- PT/OT, skin care, home health, etc.
